SURF Mentor Form

How to submit:

Students will submit this form via InfoReady. Please provide this completed form to the student.

SURF Mentor Form

Contents:

The Menor Form must be a maximum of 2 pages. If your Mentor Form is longer than two pages, everything beyond the second page will be discarded.

  • Name:
  • Department:
  • Institution: University of Arkansas, Fayetteville
  • Brief statement of research/scholarly interests. Please list recent pertinent publications or other scholarly accomplishments. New faculty and recent faculty beginning involvement in research are urged to apply. Such faculty should describe the reasons for wanting to be involved in research.
  • List the undergraduate students with project titles you have directed in research or other scholarly activity during the last five years.

Mentor's Curriculum Vitæ (CV)

How to submit:

Students will upload your Mentor CV via InfoReady. Please provide this to the student.

Letter of Support from Mentor

How to submit:

Letters of support will be submitted electronically by the mentor via Infoready for the competition. When students input their mentor's email within their InfoReady application (at the very bottom), a request with a direct link will be generated via InfoReady and sent to the mentor's email address.

The mentor letter should:

  • Address student's academic (especially research) abilities and performances.
  • Indicate student's ability to complete the project.
  • Comment on student's ability to meet the schedule and to live up to commitments.
  • Outline the mentor's advisory role, explaining what the mentor has to bring to the project.
  • Outline significance of the student's project.

Important Instructions

  • Please address all letters to the SURF Selection Committee.
  • Please make sure the letter is on university letterhead and signed.

How to submit:

Letters of support will be submitted electronically by the faculty member via Infoready for the competition. When students input their recommender's email within their InfoReady application (at the very bottom), a request will be generated via InfoReady and sent to the recommender's email address.

The mentor letter should:

  • Address student's academic (especially research) abilities and performances.
  • Indicate student's ability to complete a research project.
  • Comment on student's ability to meet the schedule and to live up to commitments.
